This service is used to obtain an Operator History Report/Confirmation Letter.
Please note that these documents cannot be emailed or sent via regular mail, they must be sent to you via registered mail.

Please complete the following fields, upload any required forms or documents (listed below), and submit your request. If there are any outstanding fines or restrictions on your file, we will contact you for further instructions.

Once all requirements are met, we will email you a credit card payment invoice. Please note that all credit card payments are subject to an additional government approved payment processing fee of approximately 3.5% of the total cost.

    * We are also required to obtain a 10-year standard driver abstract in order to complete the service, please check the box on the form for 10 Year Driver Abstract (SDA). There is an additional fee for this abstract.

  • Does not need to be notarized. Can be a valid Driver's License, Passport or Permanent Residence card. If on mobile, you can click the file upload button and take a photo with your camera or upload a photo from your camera library.

  • We will mail your original Operator History Report/Confirmation Letter via registered mail to the above mailing address for an additional $25 (within Canada) or $45 (to the USA). Please call us for international shipping fees.
    Please note that the current Government turnaround time for the letter to be produced is approximately 4-5 weeks.
